THE PROCESS

 

Homebound Services CA (HBSCA) is an access point to each service provider. This type of centralized intake process allows HBSCA to serve families or individuals, all those in need directly. No one solution fits all. However, services exist that can be leveraged to customize a plan that will have a high chance of long-term success.

 

Anyone in need of services and or housing goes to HBSCA location.

Connect with navigator for standardized intake and an assessment – The role of the navigator is to complete the initial intake assessment. In the assessment, it is found what the client needs are in services, housing, substance abuse services, mental health services, behavioral health services, or any type of social services. The navigator identifies what the client lacks to qualify for services and/or housing. Once the deficiencies are identified HomeBound Services CA personnel will work with the client to obtain proper documentation, assist in the completion of applications for housing and or services that help the client to overcome any or other types of qualifying deficiencies.

HomeBound Services CA would connect individuals, families, all those in need to the appropriate services immediately, whether that is housing, mental health, behavioral health, substance abuse care, or any other social services.

HomeBound Services CA will match the client with the proper agency and transition from Navigator to Case Manager – Once the individual relates to the appropriate agency, the agency case manager takes over in developing a long-term plan for that individual as HBSCA monitors their success.

​

HomeBound Services CA will maintain contact with the clients through the entire process to be able to then help place the client in an emergency, transitional, and/or permanent supportive housing, ultimately supplied by a Private Developer.
